The new OARS version 2 is live!

EH&S has linked to the new version of OARS from its web site as of Dec. 24, 2008. Employees, supervisors or any UW representatives can report accidents
with this new, faster, easier, and friendlier reporting system. Each computer should have current UW certificates prior to trying to report an accident.
Reporting individuals use their UW NETID to logon and auto fill many of the commonly asked questions. The first time an individual logs into the system
they are asked to verify their personal information to make sure it is correct. EH&S has added a PowerPoint presentation to our web site to help explain
how data is entered. A training database can be used to practice entering data without sending supervisors copies of the practice data. Accident data
entry, the PowerPoint presentation, the practice database, and the link to UW certificate upgrading are all found at the
EH&S web page for Accident and Incident reporting.
Under the new system supervisors, injured party and reporting party all get immediate copies of submitted reports. Supervisors have to enter the probable
causes, corrective action, and expected date of completion of the corrective action. On more serious accidents, copies are sent to EH&S and multiple layers
of supervision. Photos, emails, reports can be easily uploaded and attached for the record. This uploading feature means that when more is required a
separate document can be generated (in Word for example) and attached to the accident report file for review.
Please practice with the new OARS system. Try to be safe and not need the real site.

inspection number 312160575 at the Health Science Building has resulted in a citation for one serious and one
general violation with a fine of $4,500.00 for the electrical shock incident to an electrician. The full citation

UW Digital Security Certificate

A number of commonly used applications on the EH&S website, including but not limited to OARS Version 2
and the online general asbestos awareness training, generate errors if your computer does not have the UW
security certificate loaded. If, when attempting to access one of these applications, you get an error message
indicating that the security certificate is invalid or unknown, then go to
http://www.washington.edu/computing/ca
for instructions on how to update your computer's security certificate. This same certificate will also work for
many other University applications.

Asbestos labeling under way
